Description
The drug interacts with the platelet causing a conformational change that reveals a previously hidden platelet membrane epitope. The immune system attacks this epitope - so the antibody is against the platelet, not the drug. Testing takes washed platelets (not the patient's platelets) and tests the patient's serum for antibodies against the platelets in the presence of the drug in question. Patient's serum is tested with and without the drug, and the test is positive when the difference (or ratio) of the two reaches a particular level. This is important because it is believed to be the cause of some forms of immune thrombcytopenia and testing is usually done to assess the possibility that a given drug has caused the thrombocytopenia. However stopping the drug usually reverses the thrombocytopenia and may speak for itself. Flow cytometry is a common if not the most common way to test for this phenomenon. The culprit is mostly IgG antibodies, but IgM and IgA have also been implicated. Quinine is the exemplar for this mechanism of action, but there are tens of drugs that are known or suspected to act by the same mechanism. Penicillin can rarely cause an immune based thrombocytopenia but it covalently binds to a platelet membrane constituent so acts by a different mechanism.
