Description
The drug interacts with the neutrophil causing a conformational change that reveals a previously hidden neutrophil membrane epitope. The immune system attacks this epitope - so the antibody is against the neutrophil, not the drug. Testing takes washed neutrophils (not the patient's neutrophils) and tests the patient's serum for antibodies against the neutrophils in the presence of the drug in question. Patient's serum is tested with and without the drug, and the test is positive when the difference (or ratio) of the two reaches a particular level. This is important because it is believed to be the cause of some forms of immune neutropenia and testing is usually done to assess the possibility that a given drug has caused the neutropenia. However stopping the drug usually reverses the neutropenia and may speak for itself. Flow cytometry is a common if not the most common way to test for this phenomenon. The culprit is mostly IgG antibodies, but IgM and IgA have also been implicated. Per a lab with much experience with these tests the presence of drug dependent antibodies against WBCs are very rare compared the analogous phenomena with platelets.
