18 Beta-Glycyrrhetinic acid (GRA) is a major metabolite of glycyrrhizic acid (glycyrrhizin), which is obtained from licorice and licorice root. GRA inhibits 11 beta-hydroxysteroid dehydrogenase (11 beta OHSD) which in turn inhibits the conversion of active cortisol to inactive cortisone in the kidneys. This leads to high cortisol levels in the collecting duct of the kidney. Patients often have high blood pressure with a low renin and low aldosterone blood level. The increased amounts of cortisol binds to the unprotected, unspecific mineralocorticoid receptors and induce sodium and fluid retention, hypokalaemia, high blood pressure and inhibition of the renin-angiotensin-aldosterone system.
