Description
The "Seizure Disorder Action Plan" is a communication tool medical providers use to inform school staff of a patient's seizure disorder, treatment protocol and recommended response to seizures. School staff uses this information to guide and authorize their treatment of the child's seizure disorder while at school. The following information is included in the action plan: 1. Patient information: demographics, parent/guardian names, primary care provider/clinic name, current date, 2. Seizure Information and Signs/Symptoms (check boxes and free text): seizure type, medication, post-seizure (if need to leave classroom, student's response and return to classroom process); specifics of child's seizure condition (age onset, triggers, length/frequency, if status epilepticus history, aura, loss consciousness, etc), 3. Response recommendations: list of basic and emergency first aid actions to take in response to child's seizure activity, 4. Treatment Protocol during school hours: medication orders (type medication, dosage amount and time, special instructions and special considerations/precautions free text), and 5. Acknowledgment: medical provider and parent/guardian signatures and dates authorizing administration of medications and exchange of information/communication with school staff and provider6. Seizure Observation Record: table to use by school staff to track student seizure activity at school (demographics, date/time, length, pre-seizure, consciousness, injuries, seizure signs/symptoms, post seizure observation, if parent/guardians and/or emergency medical staff called and name of observer).
