Description

The "Anaphylaxis Action Plan" is a communication tool medical providers use to inform school staff of a patient's allergy condition and instruct how to respond if the child has an anaphylaxis event while at school. School staff uses this Information to be aware of a child's allergy condition, understand how to respond to an anaphylaxis event and authorize their treatment of the child at school for anaphylaxis. The following information is included in the action plan: 1. Patient Information: MRN, name, address, age, sex, dob, weight, parent/guardian name, if asthma, PCP/clinic name, date, school, grade, medical diagnosis, 2. Emergency contact information: names & phone numbers, 3. Allergies: asthma presence, anaphylaxis triggers, anaphylaxis symptoms, 4. Past reactions (s/s of allergic reaction): ex.s: wheezing or difficulty breathing, generalized tingling or itching, 5. Action Plan: steps to follow if Severe Symptoms (described); steps to follow if Mild Symptoms (described), medication orders: epinephrine, antihistamine and/or other meds6. Emergency Action Steps: epinephrine med order, call 911, emergency contacts, 7. Monitoring: instructions to follow after administering medications, 8. Additional health problems, 9. Pertinent medications, 10. Comments, and 11. Authorization for administration of medication away from clinic, yes or no to student self-administration, provider signature and date, parent/guardian authorization: signatures and dates consenting school to give medications, parent/guardian authorization self-administration meds by student, student agreement, school nurse signature.
