Description
Dichlorodiphenyldichloroethane (o,p'- DDD or Mitotane) is an isomer of 4,4′-Dichlorodiphenyldichloroethane (p,p'DDD) used broadly as a pesticide. as well as an oral chemotherapeutic drug used to treat Cushing's syndrome and adrenal cortical carcinoma. o,p'- DDD directly suppresses the adrenal cortex and alters the degradation of steroids. Despite the ability of o,p'- DDD to prevent excess hormone production, its side-effects are difficult to tolerate. The adverse reactions include the following:
• gastrointestinal disorders: 80% of patients endured anorexia, nausea or vomiting, diarrhea
• central nervous system: 40% of patients experience depression in the form of lethargy and somnolence (25%) and dizziness and vertigo (15%)
• skin toxicity: 15% of patients transient rashes
• infrequently occurring adverse effects include: visual blurring, diplopia, lens opacity, toxic retinopathy, hematuria, hemorrhagic cystitis, albuminuria, hypertension, orthostatic hypotension, flushing, generalized aching, hyperpyrexia, lowered protein bound iodine
