Description
Chorionic villus sampling (CVS) and amniocentesis are two methods for invasive prenatal screening and diagnosis for the fetus. CVS is usually performed during the first trimester, between 10 and 13 weeks' gestation, by using ultrasound guidance to obtain a sample of placental villi using either a transcervical or transabdominal approach. Amniocentesis is typically performed during the second trimester, between 15 and 20 weeks' gestation, using ultrasound guidance to obtain a sample of amniotic fluid. The rate of procedure-related pregnancy loss from both CVS and second trimester amniocentesis has decreased over time and is approximately 0.1-0.3% in procedures performed by experienced health care providers. Early amniocentesis, performed before 14 weeks' gestation, is associated with a higher rate of pregnancy loss, other complications, and fluid culture failures, and therefore is not recommended. Both CVS and amniocentesis samples can be used for methods that target chromosome- and gene-level variation, as well as for mitochondrial, enzyme and other biochemical analysis for the fetus. Results based on CVS and amniotic fluid samples are expected to be the same, with the exception of open neural tube defects as described below. The main advantage of CVS compared to amniocentesis is that the procedure can be performed earlier in pregnancy and the specimen processing time is shorter for the viable cells obtained by CVS, which allows earlier diagnosis and therefore more time to consider options for management.
