Descriptions

Strain is defined as the deformation of a solid object in response to an applied force. Cardiac strain has multiple components, such as circumferential, longitudinal, and radial, and strain can be either perpendicular or parallel (shear) to the force that is applied. Principal strain is the direction of greatest deformation. Strain is calculated as the change in length over a particular time period relative to the original length and is expressed as a percent. Circumferential and longitudinal strain are typically negative, meaning shortening, while radial strain is normally a positive value, indicating lengthening. During myocardial contraction, as the wall shortens, it also thickens and warrants assessment of all parameters as useful for evaluation of contractile function: positive strain (radial thickening), negative strain (circumferential shortening and longitudinal shortening). Longitudinal strains are reduced in patients with myocardial infarctions, correlate with infarct size and ejection fraction, and predict left ventricular remodeling and clinical events. Longitudinal imaging planes may include four chamber, two chamber, and long axis, which correspond to the standard apical four-chamber, apical two-chamber, and apical long-axis views used for echocardiography. Global longitudinal strain (GLS) may be generated from both speckle-tracking echocardiography and the newer semi-automated feature-tracking cardiac magnetic resonance (FT CMR) software applied to routine clinical steady-state free precession imaging. These rapid and reproducible measures of GLS may detect serial changes in myocardial function using CMR or echocardiography, which are important for following progression of disease or response to medical interventions. GLS is a sensitive predictor of prognosis for patients with cardiac amyloidosis, and impaired GLS is related to unfavorable cardiovascular events in heart failure patients.
