Description

Methotrexate (MTX) is a drug that is used to treat certain types of cancer as well as auto-immune diseases, including rheumatoid arthritis (RA), systemic lupus erythematosus and psoriasis. After uptake of MTX into cells, MTX is activated by the addition of glutamate residues and forms methotrexate polyglutamates (MTX-PGs), which are named based on the number of added glutamate residues (e.g. methotrexate triglutamate, MTX-PG3). The half-life of MTX in plasma is only several hours where as MTX PGs can persist in cells for weeks or months. PMID: 15677700 While high-dose MTX in the treatment of malignancy is due to the direct effect on tetrahydrofolate reductase and DNA synthesis, the long-term immune modulation of low-dose MTX therapy appears to be related to multiple metabolic effects of the MTX PGs. PMID: 20665136 Red blood cell MTX PG levels may be a marker for therapeutic response in patients treated with low-dose MTX, and low red blood cell MTX PG has been associated with poor clinical status in patients with RA.
