A macrolide compound obtained from Streptomyces hygroscopicus. It selectively blocks the transcriptional activation of cytokines thereby inhibiting cytokine production. It is bioactive only when bound to immunophilins. Sirolimus is a potent immunosuppressant and possesses both antifungal and antineoplastic properties. It is marketed under the drug name Rapamune (Streptomyces hygroscopicus was first isloated in a soil sample from an Easter Island called "Rapa Nui" thus the name Rapamune). Sirolimus, also known as rapamycin, is a macrolide compound that inhibits lymphocyte proliferation and is used as an immunosuppressant to prevent rejection in organ transplantation. It inhibits T-cell activation and proliferation by inhibiting the response to interleukin 2 (IL-2). Its mode of action differs from that of other immunosuppressants (SN Sehga, Clin Biochem, 31:335, 1998) most notably by not being a calcineurin inhibitor. Its inhibitory action is mediated via a cytosolic protein, FK-binding protein 12. Sirolimus is variably absorbed; levels need to monitored to achieve the therapeutic target while avoiding toxic levels. Sirolimus is marketed under the trade name Rapamune. Source: Regenstrief LOINC
