Reticulocytes are immature red blood cells (RBCs). Measures of reticulocyte maturity can provide complementary information to absolute reticulocyte counts for assessing erythropoietic activity. As reticulocytes mature, they progressively accumulate hemoglobin and lose RNA, so maturity can be measured based on the reticulocyte RNA content. Automated analyzers can use this property to subclassify reticulocytes into different maturity groups by tagging the intracellular RNA and then measuring the light scatter (fluorescence) of the tagged cells, which is proportional to the RNA content. RBCs have the least scatter, followed by the most mature reticulocytes, mid-level, and least mature, which have the highest scatter. The immature reticulocyte fraction (IRF) is defined as the proportion of mid- and high-scatter reticulocytes out of the total. A rise in IRF is one of the earliest indicators of bone marrow engraftment or recovery from intensive chemotherapy. The IRF can also be used as an early indicator of response to erythropoietin therapy in patients with chronic renal failure and to treatment for anemia. Middle (mid) light scatter (fluorescence) reticulocytes are measured to find the immature reticulocyte fraction (IRF). IRF is calculated by first dividing the reticulocytes into three categories (low-LFR, medium- MFR, and high fluorescence reticulocytes-HFR), depending on the amount of scatter they produced. The most immature reticulocytes (HFR) produce higher scatter, with the more mature reticulocytes producing the medium and low scatter. The IRF is the sum of reticulocyte fractions with medium and high fluorescence. A rise in IRF is one of the earliest indicators of bone marrow engraftment or recovery from intensive chemotherapy.
