Porcine hemagglutinating encephalomyelitis virus (PHEV) is a coronavirus found only in pigs. It has a single antigenic type and can be grown in a number of different porcine cultures. PHEV is ubiquitous and occurs in North and South America, Australia, and Asia. PHEV is found in most breeding herds of swine, where infection is typically asymptomatic and a herd immunity exists. PHEV spreads in secretions and multiplies in the nasal mucosa, tonsils, lungs, and to a lesser extent, the small intestine. PHEV enters nuclei of the medulla oblongata and then moves to the peripheral nervous system where it spreads to the brain and brain stem. Vomiting and wasting disease (VWD) leads to dehydration, cyanosis, coma and death. Both wasting and vomiting are seen primarily in pigs less than 4 weeks old. The encephalomyelitic form of PHEV infection starts with vomiting which is not severe, then moves to muscle tremors and hyperesthesia. Those symptoms are followed by blindness, opisthotonos, and nystagmus. Finally, the disease culminates into dyspnea, coma and death. Both forms of disease have mortality rates close to 100% in piglets under 4 weeks of age. Diagnosis of PHEV can be made when the virus is isolated from the brain or brain stem of euthanized pigs within 2 days of the onset of symptoms. Assays to detect viral antigens and nucleic acids have recently been developed. There is no treatment available.
