White blood cells are incubated with dihydrorhodamine 123 (DHR) and catalase, then stimulated with Phorbol 12-Myristate 13-Acetate (PMA). Dihydrorhodamine oxidation to rhodamine by the respiratory burst of the cell is measured by flow cytometry.

Neutrophil Oxidative Burst Test (or Chronic Granulomatous Disease (CGD) Test) is a measure of neutrophil oxidation is a useful assay in the diagnosis of chronic granulomatous disease and is also a useful means to determine the overall metabolic integrity of phagocytosing neutrophils. The NADPH oxidase enzyme is missing in CGD. From total blood, neutrophils can be purified and the NADPH oxidase activity can be measured with different methods in these cells after activation.

Results are expressed as a normal oxidative index (NOI), which is the ratio of the fluorescence in stimulated cells to the fluorescence expressed in unstimulated cells. The normal range is > 73 NOI. Neutrophils are white blood cells that are critical to the healthy function of the immune system. They are drawn to sites of infection and injury to phagocytose and digest microbes. PMID: 22224774 Neutrophils rely on their ability to generate an oxidative burst to effectively kill microorganisms. The oxidative burst is a series of reactions involving NADPH oxidase that is triggered by receptors when they encounter bacterial peptides such as N-formyl-Met-Leu-Phe (fMLP) and phorbol myristate acetate (PMA) and which results in the production of superoxide anions and reactive oxygen species (ROS). Chronic granulomatous disease (CGD) is an x-linked or autosomal-recessive disorder where NADPH oxidase is improperly assembled which leads to an inadequate oxidative burst response and increased susceptibility to bacterial and fungal infections. PMID: 22262438 One method of assessing neutrophil function uses flow cytometry to measure the fluorescence of neutrophils that have been stimulated in the presence of the nonfluorescent dye dihydrorhodamine 123 (DHR 123), which, when oxidized, produces the fluorescent compound rhodamine 123. Both the percent of neutrophils having a burst response and the efficacy of that response as reflected by the mean fluorescence intensity (MFI) is measured. Depending on the underlying genetic cause and severity of CGD, some patients have a population of neutrophils that can generate a burst response but with lower MFI compared to healthy controls, while others only have neutrophils that are not capable of generating a response at all.
