Description
Adrenal enzyme steroid 21-hydroxylase (P450c21, CYP21A2) lesions lead to faulty synthesis of adrenal cortisol with frequent deficiency of aldosterone. PMID: 8478006 Cytochrome P450c21 is the essential enzyme in the synthesis of cortisol. It converts progesterone and 17 alpha-hydroxyprogesterone to 11-deoxycorticosterone and 11-doxycortisol, respectively, which in turn are needed to synthesize aldosterone and cortisol. There are two CYP21 genes on chromosome 6 that are arranged together. One that is a pseudogene (CYP21A) and one that is a functional gene (CYP21B). It is proposed that gene crossover between CYP21A and CYP21B may be one of the causes of P450c21 deficiency that ultimately leads to congenital adrenal hyperplasia. One in 10,000 births are found to have P450c21 deficiency and is the primary cause of adrenal hyperplasia.
