Patients with AIED typically experience hearing loss and tinnitus over several months, but variations of the onset of AIED can include bilateral attacks of hearing loss, attacks of dizziness, and the presence of autoantibodies. Hair cells in the inner ear are commonly affected and damage to them leads to hearing loss and problems with balance. The cause of AIED is believed to be due to damage of the inner ear from autoantibodies or immune cells and cytokines due to inflammation. Viral or bacterial infections may induce the production of antibodies that cross react with inner ear antigens.https://www.dizziness-and-balance.com/disorders/autoimmune/aied.html Seventy percent of patients diagnosed with autoimmune hearing loss test positive for antibodies to the 68kD inner ear antigen and the antibody level correlates with the severity of the disease. The majority of patients with 68kD inner ear antigen autoantibodies respond well to corticosteroid treatment. (Hirose et al: The Laryngoscope 109:1769 - 1999)
