ArticleRegenstrief LOINCHuman citrullinated alpha-enolase peptide 1 is a glycolytic isoenzyme that is normally present in cells of the liver, spleen, kidney and brain. While it plays an important role in the growth of structural tissues of skin, hair follicles, and the myelin sheaths of nerve fibers, it has also been found in areas of inflammation including arthritic joints, muscle fibers, and brain tissue affected by multiple sclerosis and Alzheimer's disease. Though citrullinated alpha-enolase peptide 1 is present in a number of disease processes, it is in patients with rheumatoid arthritis (RA) that antibodies to citrullinated proteins (ACPA) are produced. These autoantibodies are 98% specific to RA, making it an excellent biomarker for the diagnosis of RA. High levels of ACPA are indicative of an increased probability of erosive disease in patients with RA. PMID: 18821669
