Description
The Rapid Plasma Reagin test (RPR) is a macroscopic flocculation test that uses cardiolipin-lecithin-cholesterol antigen coupled to carbon particles. The test detects non-specific antibodies directed against substances released by cells infected with Treponema pallidum, the organism that causes syphilis. The term "reagin" means that this test does not look specifically for antibodies against T. pallidum.
In addition to screening for syphilis, an RPR level (also called a "titer") can be used to track the progress of the disease over time and its response to therapy.
The RPR test is effective at screening patients who have syphillis but not the symptoms but in contrast, it may indicate that people have syphilis who in reality do not ( false positives). False positives can be seen in viral infections such as (Epstein-Barr, hepatitis, varicella, measles), lymphoma, tuberculosis, malaria, endocarditis, connective tissue disease, pregnancy, autoimmune diseases, intravenous drug abuse, or contamination. It can also be positive in uninfected elderly. As a result, an RPR test should always be followed up by a more specific treponemal test. Treponema pallidum hemagglutination assay (TPHA) and Fluorescent Treponemal Antibody Absorption (FTA-ABS) tests are more specific but are more expensive. Unfortunately, even with TPHA and FTA-ABS false positives can still occur in related treponomal infections such as yaws and pinta.
