The amino terminals of type III procollagen peptide (P3NP) are extensions at the ends of the type III procollagen that are cleaved during synthesis and degradation of type III procollagen to make type III collagen. P3NP are released into extracellular fluid and a marker for collagen turnover. Increased levels of P3NP occur in conditions with excessive connective tissue repair, such as fibrosis, malignant and blood-related diseases. PMID: 16679477 Source: Regenstrief LOINC , PMID: 16679477
A procollagen peptide contains an additional peptide sequence, termed the propeptide, at its amino and carboxy terminal ends. Collagen types I, II, III, IV and V are synthesized as precursor molecules called procollagens. Propeptides facilitate the winding of procollagen molecules into a triple-helix conformation within the endoplasmic reticulum. These propeptides are then cleaved from the collagen triple helix molecule during its secretion, after which the collagens polymerize into extracellular collagen fibrils. Thus, the amount of free propeptides directly correlates with collagen molecule synthesis. Procollagen Type I C-peptide (PIP) has also been extensively referenced in correlation studies between collagen levels and certain health disorders such as bone disease, alcoholic liver disease, liver cirrhosis and cirrhosis (Borrmann type IV) adenocarcinoma of the stomach. Collagen types III and I are synthesized as procollagens with a small amino terminal and a larger carboxy terminal propeptide. Once secreted into the extracellular space, the propeptides are removed by specific endopeptidases, thus allowing integration of the rigid collagen triple helix into the growing fibril. The three-amino acid procollagen type III amino terminal peptide (PIIIP) formed during this process is released into the blood.
