Paraneoplastic vision loss evaluation panel - Serum
Active
83003-4 Recoverin IgG Ab [Presence] in Serum or Plasma by Line blot
Term Description
This term was created for, but is not limited in use to, the submitter's test strip, a membrane strip pre-coated with a specific set of antigens in parallel lines. If antibodies to any of those antigens are present in the patient's serum, they will bind to the specific antigen. The bound patient antibodies are then detected using labelled anti-human antibodies. Various manufacturers classify this method as an immunoblot technique. LOINC uses the immunoblot (IB) method to mean that some electrophoretic/chromatographic separation occurs such that position (size) of the band is part of the assessment (as in Western blot). Because this assay does not involve a separation step but rather uses strips with pre-fixed antigens, the LOINC Method is Line blot.

Part Descriptions
LP222259-6 Recoverin
Recoverin is a calcium-binding photoreceptor protein that has been identified as the target of autoantibodies that results in cancer-associated retinopathy (CAR). CAR is a paraneoplastic syndrome where autoantibodies against an unknown neoplasm cross-react with retinal photoreceptor antigens and damage them leading to vision loss. PMID: 7568096 Only 65% of patients with CAR have detectable antiretinal antibodies and antibodies specific to recoverin are found in approximately 10% of those patients. Most tumors associated with CAR include small-cell lung cancer and gynecological malignancies. Neoplasms that are less often associated with CAR include bladder, prostate, pancreatic, small bowel, thymus, thyroid and colon. PMID: 26862417 Recoverin antibodies lead to cancer-associated retinopathy and have been shown to induce apoptotic death to photoreceptor cells. PMID: 12417436

- Change Reason
- This term was created for an assay that uses a membrane strip pre-coated with a specific set of antigens in parallel lines. Because the antigens are seperated by electrophoresis/chromatographic separation during manufacturing, not during the test procedure itself, this test was assigned a LOINC Method of Immunoassay (IA) and not Immunoblot (IB). However, upon further review of IA, IB, and Immunoblot strip methodologies and regulatory distinctions between these, we changed the Method from IA to Line blot so that users can distinguish between these three Methods. The meaning of the term has not changed since it was created for an IB strip assay as described in the term description.

72504-4 CV2 Ab [Presence] in Serum or Plasma by Immunofluorescence
Part Descriptions
LP32718-6 CV2
CV2 (CRMP5) antibody binds to cytoplasm and cell processes of a subpopulation of oligodendrocytes in the white matter and targets 66 kDa protein known as CRMP5.
It is ofen found in association with small cell lung carcinoma. CV2 autoimmune response can also be found in cerebellar ataxia, encephalomyelitis, limbic encephalitis, sensory and sensorimotor neuropathy, autonomic neuropathy, and chorea. CV2 autoantibodies are also present in patients with rapidly progressive dementia, and are used to monitor treatment in some cases.

- Change Reason
- The PrThr property is used for LOINC terms whose results are reported using an ordered categorical scale, regardless of whether or not an internal threshold was used to make that determination. This change was approved by the Laboratory LOINC Committee in June 2016.
