Lactate is produced via the enzyme lactate dehydrogenase(LDH) during fermentation. Elevated urine lactic acid levels are found in patients with multiple carboxylase deficiency, lactic acidosis, dihydrolipoyl dehydrogenase deficiency, PDH complex (E1, E2,E3),oxidative phosphorylation and respiratory chain defects (e.g., MERRF, MELAS, Kearns-Sayre), Crebs acid cycle defects, gluconeogenesis defects (pyruvate carboxylase, fructose-1,6-dihphosphatase, glycogen storage I disorder), short-branched chain acyl-CoA DH deficiency, severe MAD deficiency, VLCAD deficiency, GA I ,other organic acidurias (MMA,PA,IVA), citrullinemia, glycerol kinase deficiency, HMG-CoA lyase deficiency, or EMA aciduria. LP15686-6 Lactate
Lactic acid is chiral and has two optical isomers. One is known as L-(+)-lactic acid or (S)-lactic acid and the other, its mirror image, is D-(−)-lactic acid or (R)-lactic acid. The L isomer is the biologic one.
