LP15151-1   Coproporphyrin
Porphyrins build up among patients with hereditary enzyme disorders known as porphyrias. Porphyrias affect the heme biosynthetic pathway and cause increased urine excretion of porphyrinogens, by-products known as porphyrins and porphyrinogen precursors-including delta aminolevulinic acid (ALA) and porphobilinogen (PBG). Coproporphyrin and uroporphyrin excretion in urine is markedly increased during acute attacks of hereditary coproporphyria. Increased uroporphyrinogen, uroporphyrin, and coproporphyrin excretion occurs in porphyria cutanea tarda. Increased coproporphyrin excretion alone lacks specificity and sensitivity for lead testing and is likely to occur secondary to other diseases. Part of porphyrin fractionation testing useful in diagnosing porphyrin cutanea tarda, hereditary coproporphyria, and variegate porphyria. Such patients typically will present with profound elevations of porphyrin analytes (e.g. > 5-fold) during acute episodes. Creatinine or creatine anhydride, is a breakdown product of creatine phosphate in muscle. The loss of water molecule from creatine results in the formation of creatinine. It is transferred to the kidneys by blood plasma, whereupon it is eliminated by glomular filtration and partial tubular excretion. Creatinine is usually produced at a fairly constant rate and measuring its serum level is a simple test. A rise in blood creatinine levels is observed only with marked damage to functioning nephrons; therefore this test is not suitable for detecting early kidney disease. Creatine and creatinine are metabolized in the kidneys, muscle, liver and pancreas.
