The herpes simplex virus (HSV) manifests itself in two common viral infections, each marked by painful, watery blisters in the skin or mucous membranes (such as the mouth or lips) or on the genitals. The disease is contagious, particularly during an outbreak, and is incurable with present technology. An infection on the lips is commonly known as a cold sore or fever blister. These are sometimes confused with canker sores or aphthous ulcers, which have a similar appearance; these appear inside the mouth and are not caused by the herpes simplex virus. When not symptomatic HSV lies dormant in nerve cells, growing along the axons of the skin during an outbreak. When the outbreak has passed, the virus 'dies back' along the nerve until it is only present in the nerve body. The dormancy of the virus within the nerve bodies contributes to the difficulty of treatment.

Virus culture is often more sensitive than trying to detect viruses directly in clinical samples. Shell vial culture is a modification of the conventional cell culture technique for rapid detection of viruses in vitro. The technique involves inoculation of the clinical specimen on to cell monolayer grown on a cover slip in a shell vial culture tube, followed by low speed centrifugation and incubation. This system works on the principle that the low speed centrifugation enhances viral infectivity to the susceptible cells. It is thought that the minor trauma to the cell surface produced as a result of low speed centrifugation mechanical force enhances the viral entry in to the cells, which in turn reduces the total time taken for the virus to produce infection of cells. Virus may be detected by direct fluorescent antibody (DFA) or indirect fluorescent antibody (IFA) staining within hours or days of inoculation.
