Fibroblast growth factor 23, or FGF23, is primarily produced in bone and helps regulate phosphate and vitamin D metabolism by binding to receptors on renal tubular cells. It is produced as a 251-amino acid precursor consisting of a signal peptide, N-terminal domain, and C-terminal domain. The bioactive form of FGF23 consists of the N- and C-terminal domains, after the signal peptide is removed, and is also called the "intact" form. Intact FGF23 is cleaved into N-terminal and C-terminal fragments, which are no longer active, though the C-terminal fragment may compete with intact FGF23 for renal tubule receptors and play a role in regulating overall FGF23 activity. The production of FGF23 is regulated by a variety of factors, including calcium and phosphate levels, 1,25 dihydroxyvitamin D, parathyroid hormone, and iron deficiency.
