Fibroblast growth factor 23, or FGF23, is primarily produced in bone and helps regulate phosphate and vitamin D metabolism by binding to receptors on renal tubular cells. Intact FGF23 is the bioactive form that is produced after removal of a signal peptide from the inactive FGF23 precursor, and consists of an N-terminal domain and a C-terminal domain. Intact FGF23 is cleaved into N-terminal and C-terminal fragments, which are no longer active. The production of FGF23 is regulated by a variety of factors, including calcium and phosphate levels, 1,25 dihydroxyvitamin D, parathyroid hormone, and iron deficiency. Elevated FGF23 levels are associated with chronic kidney disease as well as cardiovascular disease. PMID: 24269946 Source: Regenstrief LOINC
