Creatinine or creatine anhydride, is a breakdown product of creatine phosphate in muscle. The loss of water molecule from creatine results in the formation of creatinine. It is transferred to the kidneys by blood plasma, whereupon it is eliminated by glomular filtration and partial tubular excretion. Creatinine is usually produced at a fairly constant rate and measuring its serum level is a simple test. A rise in blood creatinine levels is observed only with marked damage to functioning nephrons; therefore this test is not suitable for detecting early kidney disease. Creatine and creatinine are metabolized in the kidneys, muscle, liver and pancreas.

Buprenorphine (tradename Buprenex), is a long-lasting, high affinity opioid that acts on the same receptors as heroin and morphine but does not produce the same intense high or dangerous side effects. It is used for the treatment of pain and opioid dependence. The major buprenorphine metabolites are norbuprenorphine, buprenorphine-3-glucuronide (B3G), and norbuprenorphine-3-glucuronide (N3G). The analysis of buprenorphine and metabolites is performed to monitor compliance to drug therapy.

Norbuprenorphine is the primary active metabolite of buprenorphine, and its mean steady-state plasma concentration is comparable to or even exceeds that of buprenorphine following sublingual buprenorphine absorption (Kuhlman et al., 1998). It is likely to contribute to the overall pharmacological effects of buprenorphine. Buprenorphine and norbuprenorphine displayed distinct pharmacological properties at mu-opioid, delta-opioid, and kappa-opioid receptors and the ORL1 receptor. PMID: 11303059
