Estradiol, derived from the ovary, testis, and placenta, may be the most biologically active estrogen. It is measured in the serum to evaluate hypogonadotropic and hypoestrogenic causes of menstrual abnormalities, sexual immaturity in females, estrogen-producing tumors, and feminization in males. Estrogens are increased in liver cirrhosis and depressed in Turner's syndrome. Ingestion of estrogens, such as oral contraceptives, increases the serum levels.

Low concentrations of estradiol can be measured by high sensitivity methods for the diagnosis of hormone-related disorders in both females and males. In females, estradiol levels are useful for determining menopausal status, estrogen deficiency, monitoring anti-estrogen treatment, and predicting risk of osteoporosis and cardiovascular disease. In males, estradiol levels are useful for evaluation of feminization (e.g. gynecomastia) and diagnosis of estrogen-producing neoplasms. Estradiol levels are also important for diagnosing precocious and delayed puberty in females, and, to a lesser degree, males. PMID: 20077073
