Humans are considered the only hosts of the nematode Enterobius vermicularis which are small, white, threadlike worms. Females possess a long, pin-shaped posterior end, hence the name "pinworm." Enterobius vermicularis dwell primarily in the large intestine, from where the gravid females migrate at night to lay up to 15,000 eggs on the perineum. A second species, Enterobius gregorii, has been reported from Europe, Asia and Africa and has identical morphology, life cycle, clinical presentation and treatment. Enterobiasis is the most common helminthic infection (est. 40 million persons infected) in the United States. Worldwide enterobiasis appears more common in temperate countries and infections worldwide are more frequent in children and in crowded conditions. Embryonic eggs are typically ingested by humans. Self-infection occurs by transferring infective eggs to the mouth with hands that have scratched the perianal area. Transmission also occurs through handling of contaminated clothes or bed linens or through other surfaces contaminated with pinworm eggs. Enterobiasis is frequently asymptomatic with the most common symptom being perianal pruritus. The female genital tract may also become infected. Other symptoms may include abdominal pain, anorexia and irritability. Microscopic identification of eggs collected in the perianal area is the method of choice for diagnosis. Enterobiasis (pinworm infection) can be diagnosed by visualization of the worms in the perianal area or by using the transparent tape method, in which transparent tape is pressed to the perianal area, applied sticky side down to a slide, and then examined under the microscope for the presence of pinworm eggs. The tape collection should be done as soon as the person with suspected infection wakes up in the morning, before they stand up or do any washing. Specimens should be collected on 3 consecutive mornings, and then the slides should be taken to the healthcare provider for examination under a microscope.
