Morphine is an extremely powerful opiate analgesic drug and is the principal active agent in opium. Like other opioids such as heroin, morphine acts directly on the central nervous system (CNS) to relieve pain, in particular by targeting the synapses of the arcuate nucleus. Side effects include impairment of mental performance, euphoria, drowsiness, lethargy, and blurred vision. It also decreases hunger, inhibits the cough reflex, and produces constipation. Morphine is highly addictive when compared to other substances, and tolerance and physical and psychological dependence develop quickly. Patients on morphine often report insomnia, visual hallucinations and nightmares.

Oxymorphone (Numorphan) or 14-Hydroxydihydromorphinone is a powerful semi-synthetic opioid analgesic derived from morphine; it is 6-8 times more potent. Clinically, it is administered as its hydrochloride salt via injection, or suppository; typically in dosages of 1 mg (injected) to 5 mg (suppository). As with other opioids, oxymorphone can cause physical dependency, and may be abused.

Codeine is a plant alkaloid of opium that relieves acute or chronic pain and suppresses cough. [https://pubchem.ncbi.nlm.nih.gov/compound/codeine#section=Top] Codeine is extensively prescribed throughout the world and is frequently combined with non-opiate drugs including acetaminophen and aspirin. Codeine is a tenth as potent as morphine and is considered its prodrug. A small portion of codeine is metabolized by the enzyme CYP2D6 to morphine which is responsible for the analgesic affect of codeine. Oxycodone is a potent and addictive opioid analgesic medication synthesized from thebaine. Its name is derived from codeine, which has a very similar chemical structure. It is effective orally and is marketed in combination with acetylsalicylic acid (Percodan, Endodan, Roxiprin) or paracetamol (Percocet, Endocet, Roxicet, Tylox) for the relief of pain. More recently, ibuprofen has been added to oxycodone (Combunox). Long term users may develop a tolerance or resistance to the drug. Abusers crush the tablets and injest the powder orally, intranasally, or via intravenous, intramuscular or subcutaneous injections.

Synthetic opiates based on the phenanthrene nucleus of morphine include hydromorphone, hydrocodone, oxymorphone, oxycodone, dihydrocodeine, meperidine, and levorphanol. Opium comes in chunks and powder and may be eaten or smoked. Heroin is a manufactured opiate and is made into a powder that may be dissolved in water and injected.
