Acute promyelocytic leukemia (APL) involves the t(15;17)(PML;RARα )(q22;q12) translocation occurring between the promyelocytic leukemia (PML) gene and the retinoic acid receptor - alpha (RARα) gene, leading to expression of the PML- RARα fusion protein. This test detects the presence of this translocation by examining the subcellular distribution of PML by Immunofluorescence (IF). Normally, IF staining for PML using antibodies directed against the PML protein appears as a granular pattern of 5 to 30 discrete intranuclear particles. This is the case in normal cells. However, in acute promyelocytic leukemia, the PML- RARα fusion protein forms heterodimers with the wild type PML protein. This sequestration changes the cellular localization of PML such that IF staining becomes more diffuse, known as a microgranular pattern.
