Oral fluid is broadly used to test for drugs of abuse in employment and forensic settings, and is especially useful since it is difficult to adulterate, is quick and is non-invasive. Oral fluid is used to test a variety of analytes, including to test alcohol, HIV and other antibodies, therapeutic drugs and steroids, but its primary use is to test for drugs of abuse including amphetamines, cocaine and metabolites, opioids, methadone, cannabis and heroin. The concentration of tetrahydrocannabinol (THC) in oral fluid is similar to that of plasma, though in cases where recent ingestion has occurred, levels may be higher due to the significant local absorption in the oral cavity. Confirmatory testing on oral fluid using liquid chromatography-mass spectrometry (LC-MS) is highly sensitive and specific even on relatively small sample sizes. An increased understanding of pharmacokinetics in oral fluid has flourished inover the last ten years has allowed the correlation of .
