The recent outbreak (2012) of fungal meningitis in the United States has been attributed to contamination of epidural steroid injection, methylprednisolone acetate by a rare fungal pathogen, Exserohilum rostratum. As of November 4, 2012, 400 cases, which include 7 peripheral joint infections, have been reported in 19 states. Twenty-nine of the cases have resulted in death. It is estimated that approximately 14,000 patients received contaminated steroid injection for pain treatment. The pathogen E. rostratum has been reported in clinical specimens from multiple cases with fungal meningitis and other spinal infections. Real time PCR assays are used for the detection of Exserohilum DNA from multiple clinical specimen types. The high specificity and sensitivity of real time PCR assays assist in early diagnosis and the selection of optimal treatment regimens, reducing the mortality and morbidity associated with E. rostratum-induced meningitis or other ailments.
