Phenobarbital is an antiepileptic drug first introduced for clinical use in 1912. It is still used to treat most seizures except for absence seizure types. The CYP2C19 enzyme metabolizes phenobarbital to p-hydroxyphenobarbital which is then excreted as glucuronide. Phenobarbital induces secretion of liver enzymes which can impact the efficacy of drugs administered at the same time. The most common side effect of phenobarbital in adults with blood levels greater than 40 mg/L is sedation. Phenobarbital is an active metabolite of primidone and so must also be monitored in patients on primidone.(Tietz Fundamentals of Clinical Chemistry and Molecular Diagnostics, Seventh Edition; Carl A.
