This assay measures the amount of adalimumab antibodies. In the absence of anti-adalimumab antibodies, the adalimumab drug level typically reflects the total adalimumab concentration in serum. In the presence if anti-adalimumab antibodies, the adalimumab level typically reflects the antibody-unbound fraction of adalimumab concentration in serum. At drug levels of 2 ug/mL or higher, anti-adalimumab antibody quantification may be affected. However, even at adalimumab drug levels of 100 ug/mL, positive anti-adalimumab levels remain detectable.

Adalimumab is a human monoclonal antibody that binds tumor necrosis factor alpha (TNFα). TNFα is a cell signaling protein that, when present in excess, may lead to the development of inflammatory immune diseases including Crohn's disease, rheumatoid arthritis, psoriatic arthritis and inflammatory bowel disease. Adalimumab inactivates TNFα by binding to the TNFα receptor. PMID: 23943614 Treatment with adalimumab does not necessitate laboratory monitoring though association with increased transamines and pancytopenia may argue for intermittant blood cell counts and liver function tests. Adalimumab has been associated with serious infections including the reactivation of tuberculosis and deep fungal infections as well as other rare side effects, including lymphoma, heart failure and demyelinating disease. Antibodies to adalimumab were shown to develop in 67% of patients within the first 28 weeks of treatment. Only thirteen percent of patients with adalimumab antibodies experienced relief from symptoms compared to 48% of patients without antibodies to adalimumab. Only 4% of patients with antibodies went into remission compared to 34% of patients who achieved remission without anti-adalimumab antibodies.
