Term Description

The Patient Examination portion of the General Practitioner assessment of Cognition (GPCOG) is a cognitive assessment for recall, time orientation, clock drawing, and ability to provide current news information. PMID: 11943052 The screening tool is designed for primary care settings. Scores between 0-9 where 9 indicates no significant cognitive impairment (all answers are correct), 5-8 is equivocal, and 0-4 indicates cognitive impairment. If the patient scores 5-8, the patient's informant completes the informant questionnaire [LOINC: 79116-0]. If the patient scores 0-3 out of 6 on the informant questionnaire, cognitive impairment is indicated.
