Dried blood spot (DBS) specimens have been used for therapeutic drug monitoring (TDM) across several classes of drugs including antimicrobials, anticonvulsants and immunosuppressants. DBS specimens are useful for TDM in infants and children due to the small sample volume required. DBS specimens are also useful for outpatient TDM in adults who have been trained to collect their own DBS because most analytes are relatively stable in DBS and the DBS card does not require specialized transportation. Trimethoprim is a synthetic (man-made) antibiotic that interferes with the production of tetrahydrofolic acid, a chemical necessary in protein production. The drug inhibits the enzyme responsible for making tetrahydrofolic acid from dihydrofolic acid. Trimethoprim inhibits the bacterial enzyme more than the corresponding human enzyme, so it has less effect on the production of tetrahydrofolic acid by humans.
