The projected heparin concentration is the amount of heparin required to increase the patient's ACT to the clinician specified target value and is based the target ACT and the slope of the patient's response to known amounts of heparin. This term was created for, but is not limited in use to, the submitter's Medtronic Heparin Dose Response test kit.

The projected heparin dose response test is a point-of-care test performed on fresh whole blood (no citrate added) to assess patient's individual response to heparin. The test measures the baseline (no heparin) activated clotting time, and the activated clotting times on the same blood to which 2 different concentrations of unfractionated heparin is added. From these measurements a projected dose response is calculated. This test is used in the cardiac operating room where patients are anticoagulated with high (3 to 5 units heparin/mL of blood) levels of unfractionated heparin. The effect of this level of heparin anticoagulation is measured every 20 to 30 minutes while the patient is on cardiopulmonary bypass. The heparin dose response is used to determine the appropriate amount of heparin needed to increase the patient's activated clotting time to the target level used by the hospital. The heparin dose response test is typically performed once at the beginning of the surgical procedure before the patient is placed on bypass. Once the patient is heparinized and placed on bypass the activated clotting time and the heparin concentration (whole blood heparin assay) are determined in the operating room every 20 to 30 minutes during the procedure. Heparin administered to patients in the cardiac OR is unfractionated, not low molecular weight. The amount given greatly exceeds the amount that could be monitored by the APTT. In this setting the most important concern is that the patient receive the appropriate amount in order to reach and maintain the required anticoagulant (thrombin inhibiting) protection.
