Murray Valley encephalitis virus (MVEV) is a zoonotic flavivirus endemic to northern Australia and New Guinea and is the causal agent of Murray Valley encephalitis. MVEV is related to West Nile virus Kunjin strain but is more virulent for humans compared to West Nile Kunjin. The majority of MVEV infections are asymptomatic or cause a non-specific febrile illness that may be associated with headache, myalgias and rash, but approximately 1 out of 150 to 1000 MVEV infections result in clinical encephalitis. The incubation period is one to four weeks, and patients with encephalitis usually present with fever, headache, malaise, and altered mental status. Complications include progressive neurological deterioration, parkinsonian tremor, cranial nerve palsies, peripheral neuropathy, coma, and flaccid paralysis. The reported case fatality rate of human MVEV encephalis is between 15-30%. PMID: 24466360 Laboratory confirmation of a suspected MVEV infection requires either the isolation of the virus itself, detection of MVEV RNA, a fourfold rise in the IgG titer between acute and convalescent serum samples, or detection of IgM in serum or CSF (in the absence of cross-reactive antibodies).
