Term Description
Angiography is one method for determining the degree of mitral valve regurgitation. The Sellers grading system, originally published in 1964, is based on the opacification of the left atrium and is still used today. The Sellers grading system is as follows: 1) + = a regurgitant jet is visible with minimal, rapidly clearing staining of the left atrium; 2) ++ = evidence of a regurgitant jet with moderate but still rapidly clearing opacification of the left atrium; 3) +++ = the left atrium is as opaque as the left ventricle and aorta on the late films and clears slowly. A jet usually is not seen and the left atrium is usually significantly enlarged; and 4) ++++ = the left atrium is more densely opacified than the left ventricle or the aorta. The left atrium is usually
markedly enlarged and the left ventricle is dilated. The left atrium remains intensely opacified on the whole series of films. PMID: 14215054

Regurgitation (also known as backflow) is a heart valve problem where, if a valve doesn't close tightly, blood leaks back into the chambers rather than flowing forward through the heart or into an artery. Regurgitation, especially that of the mitral valve, is often due to prolapse, which is when the leaflets of the valve flop or bulge back into the heart chamber during systole.
