Term Description

The sum activity of peripheral deiodinases (GD, also referred to as deiodination capacity or total deiodinase activity) is the maximum amount of triiodothyronine (T3) produced per time-unit by deiodinases outside the central nervous system. The structure parameter inference approach (SPINA) uses Michaelis-Menten functions and pharmakokinetic data to estimate GD under conditions of substrate saturation. One calculation is based on peripheral free T3 and free T4 values, and an alternate calculation uses peripheral total T3 and free T4. GD calculated using the SPINA approach is also known as SPINA-GD. SPINA-GD may be useful in diagnosing thyroid conditions that are difficult to diagnose based on traditional thyroid function studies, such as subclinical forms of thyroid failure, hypothalamic or pituitary dysfunction, and non-thyroidal illness syndrome. Peripheral deiodinase activity is the rate of conversion of circulating T4 to the biologically active T3 by monodeiodination. Changes in peripheral deiodinase activity may reflect conditions that are difficult to diagnose based on traditional thyroid function tests, such as non-thyroidal illness syndrome. PMID: 27375554

Estimated sum activity of peripheral deiodinases (GD) represents deiodination capacity, i.e. the maximal amount of T3 that can be formed from T4 in one second. SPINA-GD is calculated with GD = beta31 * (KM1 + [FT4]) * (1 + K30 * [TBG]) * [FT3] / (alpha31 * [FT4]) or GD = beta31 * (KM1 + [FT4]) * [TT3] / (alpha31 * [FT4]) from serum or plasma concentrations of free (FT3) or total (TT3) triiodothyronine and free (FT4) or total (TT4) thyroxine, and some constants (alpha31 = Dilution factor for T3: 0.026 l-1; beta31 = Clearance exponent for T3: 8e-6 sec-1; KM1 = Dissociation constant of type-1-deiodinase: 5e-7 mol/l; K30 = Dissociation constant T3-TBG: 2e9 l/mol) Source: Regenstrief LOINC
