Borrelia miyamotoi is a spirochetal bacterium that is closely related to the Borrelia species that causes tick-borne relapsing fever (TBRF), and it is more distantly related to the Borrelia species that cause Lyme disease. First identified in 1995 in ticks from Japan, B. miyamotoi has since been detected in two species of North American ticks, the black-legged or "deer" tick (Ixodes scapularis) and the western black-legged tick (Ixodes pacificus). These ticks are already known to transmit several diseases, including Lyme disease, anaplasmosis, and babesiosis. Patients with B. miyamotoi infection are most likely to have fever, chills, and headache. Other common symptoms included body and joint pain and fatigue. Unlike Lyme disease, rash is uncommon, presenting in only 4 out of 51 patients. Patients infected with B. miyamotoi have been treated with a 2-4 week course of doxycycline, amoxicillin, or ceftriaxone. Fewer than 60 well-documented cases of human infection have been reported in the United States as of July 2015. The route of transmission of B. miyamotoi to humans has not been firmly established, but likely involves the bite of infected black-legged ticks.
