Health literacy and communication domain risk level [ePCAM]
The risk level is calculated as the two question response total (out of a possible 200) [LOINC: 85450-5] divided by 200, then multiplied by 100 to express the risk as a percent. The ePCAM guidance recommends "Routine Care" for a risk level of 0-25%, "Active Monitoring" for 26-50%, "Plan Action" for 51-75%, and "Act Now" for 76-100%.
