Term Description
The detection of carbapenemase-producing isolates by various phenotypic methods, including the Modified Hodge Test (MHT) and Carbapemenase Inactivation Method (CIM). In MHT, carbapenemase production is detected when the test isolate produces the enzyme and allows growth of a carbapenem susceptible strain (E.coli ATCC 25922) towards a carbapenem disk. The result is a characteristic cloverleaf-like indentation. For CIM, a disk containing the indicator antibiotic meropenem is incubated for 2 hours in a bacterial suspension of the isolate. When the isolate produces carbapenemase, the meropenem in the disk is inactivated allowing uninhibited growth of an indicator strain. PMID: 28476242

Carbapenems are famously stable to AmpC β-lactamases and extended-spectrum-β-lactamases. Carbapenemases are a diverse group of β-lactamases that are active not only against the oxyimino-cephalosporins and cephamycins but also against the carbapenems. Carbapenemases were formerly believed to derive only from classes A, B, and D, but a class C carbapenemase has been described.
