Cytolethal distending toxin B (CdtB) is a protein that is produced by gram negative bacteria. CdtB targets eukaryotic DNA. PMID: 16427513 Antibodies against this toxin can cross react with vinculin, a cytoskeletal protein found in epithelial cells. In animal models, these vinculin antibodies have been shown to cause symtoms similar to irritable bowel syndrome (IBS). CdtB antibodies are useful as biomarkers for irritable bowel syndrome with diarrhea (D-IBS) in humans, and may help distinguish D-IBS from inflammatory bowel disease in the workup of chronic diarrhea. Cytolethal distending toxin B (CdtB) and vinculin antibodies are useful as biomarkers for irritable bowel syndrome with diarrhea (D-IBS) in humans. Studies suggest that elevated levels of vinculin antibodies are more commonly found in patients with D-IBS than those with other forms of inflammatory bowel disease (IBD) and may help distinguish the two. PMID: 25970536 This term was created for, but is not limited in use to, the Quest Diagnostics' IBSDetex assay.
