Dermatan sulfate is a derived disaccharide that accumulates in lysosomes, extracellular matrix, and body fluids of people with mucopolysaccharidoses (MPSs). MPSs are rare disorders caused by a deficiency of enzymes that break down glycosaminoglycans (GAGs). Symptoms of MPS vary due to which storage product accumulates and the level of enzymatic function or concentration that remains. Symptoms include musculoskeletal, airway, cardiac, and central nervous system disorders. Treatments include hematopoietic stem cell transplantation for the more critical forms of MPS and intravenous enzyme replacement therapy for the others. Treatment is most effective when it is initiated before irreversible tissue damage has occurred. Because MPS is so rare and the symptoms so variable, it is difficult to diagnose in its earliest stages. A solution to that problem is the development of newborn screening assays that can detect dermatan sulfate, though the levels in the dried blood spot samples do not appear to predict the severity of disease.
