Trimethylamine N-oxide (TMAO) is a colorless amine oxide that is produced from choline, betaine, and carnitine by gut microbial metabolism. Recently, elevated plasma TMAO levels in humans have been associated with risk for serious adverse cardiovascular incidents and death. Diet, the composition of gut microbial flora, and liver flavin monooxygenase activity all affect the plasma levels of TMAO. Foods that contain choline, lecithin, and carnitine, including red meat, eggs, dairy products, and salt-water fish, are sources of TMAO. TMAO alters cholesterol and bile metabolism, activates inflammatory pathways, and promotes foam cell formation. It is strongly suspected in mediating the development of type-2 diabetes mellitus. TMAO also increases with decreasing kidney function, and is associated with an increased risk of death in patients with chronic kidney disease. Treatments being explored to lower TMAO include broad-spectrum antibiotics to eliminate TMAO producing bacteria, the promotion of the growth of bacteria that utilize TMAO as a substrate, and the development of target-specific molecules.
