Polymorphic DNA markers are used to determine if twins (triples, etc.) are monozygotic (identical) or dizygotic (fraternal). In some cases, labs may require DNA from the parents to ensure markers are informative and provide the most accurate results. Genotypes derived from the analysis of the markers are compared between the siblings (and parents). Any differences indicate dizygosity. If siblings share the same genotypes, they are considered identical.

Zygosity testing is performed for several reasons, including determining genetic risk for an individual whose sibling is affected with a genetic disorder for which a specific genetic test is uninformative or not available, assessing risks prenatally when one fetus of multiples is known to be affected by a specific disorder, performing organ or bone marrow transplantation compatibility testing, evaluating twin-twin transfusion syndrome during pregnancy, and for general familial or parental interest.
