Methemoglobin is a form of the oxygen-carrying protein hemoglobin, in which the iron in the heme group is in the Fe3+ state, not the Fe2+ of normal hemoglobin. Metheomglobin appears blue and is unable to carry oxygen. The NADH-dependent enzyme methemoglobin reductase (AKA diaphorase I) is responsible for converting methemoglobin back to hemoglobin. Normally 1-2% of people's hemoglobin is methemoglobin, a higher percentage than this can be genetic or caused by exposure to various chemicals and depending on the level can cause health problems. A higher level of methemoglobin will tend to cause a pulse oximeter to read closer to 85% regardless of the true level of oxygen saturation. Methemoglobinemia can be caused by genetic abnormalities such as diaphorase I deficiency, or abnormal hemoglobin variants. It can also be caused by certain oxidizing drugs (benzocaine), antibiotics (trimethoprim or sulfanamides), analine dyes, metoclopramide, chlorates, bromates, and nitrates. Infants under 6 months are particularly susceptible when exposed to nitrates, benzocaine, or even dehydration. Methemoglobinemia can be treated with supplemental oxygen and methylene blue.

The composition of central venous blood is different from mixed venous blood, found in the pulmonary artery and also sampled via catheter, and peripheral venous blood, sampled via venipuncture. Central venous blood contains blood from the upper body, whereas mixed venous blood contains deoxygenated blood from the entire body.
