Bacterial vaginosis (BV) is the most common cause of vaginal infection. It is less commonly referred to as vaginal bacteriosis. It is not considered to be a sexually transmitted infection. BV is not transmitted through sexual intercourse but is more common in women who are sexually active. BV is caused by an imbalance of naturally occurring bacterial flora and should not be confused with yeast infection (candidiasis), or infection with Trichomonas vaginalis (trichomoniasis), which are not caused by bacteria.

Trichomonas vaginalis (TV) is an anaerobic, flagellated parasitic protozoan that is among the most common non-viral sexually transmitted organisms whose only known host is humans. It is estimated that 170 million new cases occur worldwide and it is the most common sexually transmitted diseases in the United States. TV is known to increase HIV acquisition and transmission in both men and women by acting as a cofactor to increase susceptibility to HIV of an uninfected partner and infectivity of HIV of an infected partner. In women, infection with TV is a possible risk factor for developing cervical neoplasia, post-operative infections, adverse pregnancy outcomes, atypical pelvic inflammatory disease and infertility. In men, infection with TV can lead to non-gonococcal urethritis, infertility, chronic prostatitis and decreased sperm motility. Diagnosis is most often made using wet mount microscopy which has a sensitivity of only 52%. Other diagnostic methods include Pap smear, culture and PCR with sensitivities of 56%, 78% and 84%, respectively. There is an estimated 5 million new cases of TV annually in the United States and yet TV is still not a reportable disease. Treatment for TV is with metronidazole though emerging metronidazole-resistant strains have been reported. Asymptomatic infection can persist for months to years. Previously known as Torulopsis glabrata, Candida glabrata is a fungal saprophyte that is part of the normal flora of human mucosal tissues and is usually nonpathogenic. Unlike other species of Candida, C glabarata has a nondimorphic blastoconicial morphology and a haploid genome. C glabrata often is implicated in superficial infections of the mouth, esophagus, vagina, urinary tract, and in systemic candida infections. C glabrata infection has become common in patients on immunosuppressive therapy and in patients with diabetes. It can be difficult to treat since it is resistant to a number of antifungal agents including fluconazole. As a result, immunocompromised patients have a high mortality rate when infected with C glabrata. PMID: 9880475

Chlamydia trachomatis is one of three bacterial species in the genus Chlamydia, family Chlamydiaceae, class Chlamydiae. C. trachomatis has only been found living inside human cells . It causes urethritis , epididymitis, and proctitis in men and cervicitis, salpingitis, urethritis and proctitis in women. C. trachomatis has also been detected in some patients with temporomandibular joint (TMJ) disease. It may be treated with any of several antibiotics such as azithromycin, erythromycin or doxycyclinetetracycline. C. trachomatis was the first chlamydial agent discovered in humans. It comprises two human biovars: trachoma and lymphogranuloma venereum (LGV). Many, but not all, C. trachomatis strains have an extrachromosomal plasmid.

Neisseria gonorrhoeae are gram-negative cocci usually seen in pairs with the adjacent sides flattened. Pili, filament-like appendages, extend from the cell surface and have a role in adherence. Gonorrhea is usually acquired by sexual contact. Men and women aged 15-29 have the highest incidence of gonorrhea. Number of sexual partners, sexual practices and preference, condom use and population mobility contribute to disease incidence. The most frequent sites are the cervix, urethra, rectum, pharynx and conjunctiva. The most common symptom in men is discharge that may be scanty and clear or cloudy to copious and purulent, and often dysuria. Asymptomatic men are an important reservoir for transmission. Endocervical infection is the most common form of infection in women. Ocular infections occur most commonly in newborns who are exposed in the birth canal and can lead to corneal scarring or perforation. Gonococcal bacteremia may lead to disseminated infection including pelvic inflammatory disease in as many as 15 percent of women, leading to an increased probability of infertility and ectopic pregnancy. While antibiotics have successfully treated gonorrhea for decades, N. gonorrhoeae has developed antimicrobial resistance to every drug used for treatment. The Centers for Disease Control and Prevention recommends combination therapy using two antimicrobials with different mechanisms of action (e.g., a cephalosporin plus azithromycin) to improve treatment efficacy and potentially slow the spread of resistance to cephalosporins.
