85618-7 Physical score [MLHFQ]

Term Description

The physical subscore reported as part of the Minnesota Living with Heart Failure (R) Questionnaire is calculated by adding the numeric responses to 8 specific self-assessments regarding whether or not a patient's heart failure prevented them from living as they wanted during the past month (4 weeks) from a physical perspective. The 8 assessments used (by question number in survey) are: 2 - making you sit or lie down to rest during the day; 3- making your walking about or climbing stairs difficult; 4 - making your working around the house or yard difficult; 5 - making your going places away from home difficult; 6 - making your sleeping well at night difficult; 7 - making your relating to or doing things with your friends or family difficult; 12 - making you short of breath and 13 - making you tired, fatigued, or low on energy.

The responses are based on a scale of 0-5 where 0 = No, 1 = Very Little and 5 = Very Much; therefore, the physical subscore is out of a total of 40 points, with a higher score indicating increasing difficulty.

Form Coding Instructions

If one or more responses are missing at baseline, then assign a zero for the missing baseline value(s) and carry the zero value(s) forward to every subsequent questionnaire completed by that subject regardless of whether he/she answers the question after the baseline assessment. If a response is available at baseline, but subsequently missing, then carry the baseline response forward to complete the missing value. These rules will neutralize the effects of missing values when calculating and comparing changes in scores.

85609-6 Emotional score [MLHFQ]

Term Description

The emotional subscore reported as part of the Minnesota Living with Heart Failure (R) Questionnaire is calculated by adding the numeric responses to 5 specific self-assessments regarding whether or not a patient's heart failure prevented them from living as they wanted during the past month (4 weeks) from an emotional perspective. The 5 assessments used (by question number in survey) are: 17 - making you feel you are a burden to your family or friends; 18 - making you feel a loss of self-control in your life; 19 - making you worry; 20 - making it difficult for you to concentrate or remember things and 21 - making you feel depressed.

The responses are based on a scale of 0-5 where 0 = No, 1 = Very Little and 5 = Very Much; therefore the emotional subscore is out of a total of 25 points, with a higher score indicating increasing difficulty.

71938-5 Total score [MLHFQ]

Term Description

The total score in the Minnesota Living With Heart Failure Questionnaire is the sum of the responses to all 21 questions. Based on the 6-point, zero to five, Likert scale to show how much each of 21 life characteristics prevented them from living as they desired, the lower the score, the less the patient feels their heart failure is impacting their life. One study was able to identify three levels of Quality of Life (QOL) and determined that a score of less than 26 indicated a good QOL, between 26 and 45 a moderate QOL, and greater than 45 a poor QOL.
