Term Description
This term was developed in collaboration with the National Institute of Health's National Eye Institute (NEI) as part of an effort to standardize the data elements collected by the National Ophthalmic Disease Genotyping and Phenotyping Network (eyeGENE).
Source: Regenstrief LOINC
Part Descriptions
LP200585-0 a-wave amplitude
The a-wave is an electroretinographic measurement of the initial corneal-negative deflection, derived from the cones and rods of the outer photoreceptor layers. This wave reflects the hyperpolarization of the photoreceptors due to closure of sodium ion channels in the outer-segment membrane. Absorption of light triggers the rhodopsin to activate transducin, a G-protein, which leads to the activation of cyclic guanosine monophosphate phosphodiesterase (cGMP PDE) and eventually to a reduction in the level of cGMP within the photoreceptor. Reduction in cGMP leads to closure of the sodium ion channels, which results in a decrease of inwardly directed sodium ions, or hyperpolarization of the cell. The a-wave amplitude is measured from baseline to the trough of the a-wave.

LP200597-5 dark-adapted
Electrooculography (EOG) is a technique for measuring the resting potential of the retina. The EOG is used to assess the function of the pigment epithelium. During dark adaptation, the resting potential decreases slightly and reaches a minimum ("dark trough" or "Dt") after several minutes. When the light is switched on, a substantial increase of the resting potential occurs ("light peak" or "Lp"), which drops off after a few minutes when the retina adapts to the light. The ratio of the voltages (i.e., light peak divided by dark trough) is known as the Arden ratio and is normally above 1.80. The Arden ratio is often abnormal in retinitis pigmentosa, diabetic retinopathy, detached retina, Best disease and many other eye disorders.

LP200586-8 b-wave amplitude
The b-wave is an electroretinographic (ERG) measurement of the corneal-positive deflection, derived from the inner retina (predominantly Muller and ON-bipolar cells). The hyperpolarization of the photoreceptor cells results in a decrease in the amount of neurotransmitter released, which subsequently leads to a depolarization of the post-synaptic bipolar cells. The bipolar-cell depolarization increases the level of extracellular potassium, subsequently generating a transretinal current. It is this transretinal current that depolarizes the radially oriented Muller cells and generates the corneal-positive deflection. The b-wave amplitude is generally measured from the trough of the a-wave to the peak of the b-wave. This wave is the most common component of the ERG used in clinical and experimental analysis of human retinal function.

Term Description

Part Descriptions
LP200587-6 a-wave implicit time
Implicit a-wave time is the amount of time between the flash onset and the trough of the retinal a-wave produced on an electroretinogram (ERG).
Source: Regenstrief LOINC

96201-9 Right optic disc and retinal nerve fiber layer Function by ERG
Part Descriptions
LP248912-0 Retinal nerve fiber layer
The RNFL is formed from expansion of the fibers of the optic nerve and extends to the interior surface of the retina. The RNFL is divided into 4 quadrants when measured in each eye - temporal, superior, nasal, and inferior. Each 90 degree quadrant is further divided into three 30 degree sectors ("clock hours"). RNFL measurements include the thickness of each quadrant and the thickness of each sector. The mean thickness of the entire RNFL for each eye is calculated based on the individual measurements. In patients with glaucoma, RNFL changes in the inferior quadrant are most closely associated with glaucoma status. In addition, superior visual field defects, which correspond to the inferior RNFL quadrant, are usually the first to appear. PMID: 10636409
